Tunnels at old State Hospitals are for the steam pipes from the Power House. They run to all the buildings, also for electric. Nothing to do with housing patients. Or transporting them.
I know of many installations that are connected by tunnels, originally used for the steam pipes as you say, but when the weather is 20 and 30 degrees below zero I would opt to go from building to building via the tunnels rather than going outside. I would suspect that some places may have occasionally used them for transport in times of inclement weather.
